Equine osteoarthritis, commonly referred to as degenerative joint disease (DJD), is a chronic and progressive condition affecting horses. It is characterized by the breakdown of articular cartilage, the smooth, protective tissue covering the ends of bones within a joint. This degradation initiates a complex inflammatory response within the joint, involving the synovial membrane, subchondral bone, and supporting ligaments. The ultimate result is pain, stiffness, and a decline in the horse's mobility and athletic performance.

Pathophysiology of Articular Cartilage Degradation

The primary event in osteoarthritis is the damage and eventual loss of articular cartilage. This can be caused by direct trauma, repetitive stress from athletic activities, developmental abnormalities, or systemic factors. Initially, the cartilage matrix degrades, leading to a loss of its smooth surface and shock-absorbing capabilities. The underlying subchondral bone can also be affected, leading to increased density and the formation of new bone at the joint margins, known as osteophytes. Inflammation of the synovial membrane (synovitis) is a common secondary event, resulting in increased production of inflammatory cytokines and enzymes that further contribute to cartilage breakdown. The synovial fluid may become thicker and less effective as a lubricant.

Types and Subtypes of Equine Osteoarthritis

Osteoarthritis in horses can be broadly categorized based on its cause:

  • Primary Osteoarthritis: This form is less common in horses and is thought to be related to aging and wear-and-tear on joints without a specific inciting event.

  • Secondary Osteoarthritis: This is the most common type in horses and develops as a consequence of other factors, including:

  • Joint Instability: Ligament injuries or joint instability can lead to abnormal joint mechanics and accelerated cartilage wear.

  • Infection (Septic Arthritis): While acute, septic arthritis can lead to significant joint damage and subsequent chronic osteoarthritis if not treated effectively.

  • Systemic Diseases: Certain metabolic or endocrine disorders can influence joint health, though these are less common causes of primary osteoarthritis in horses.

Recognizing Signs of Degenerative Joint Disease in Horses

Lameness is the hallmark sign of osteoarthritis in horses. The nature of the lameness can vary significantly, often being worse after periods of rest or upon starting work, and improving as the horse 'warms up.' This characteristic is known as a warming-up lameness.
