In dogs, this is most commonly caused by pancreatic acinar atrophy (PAA), a progressive decline in healthy tissue that leaves the digestive system unable to break down macronutrients into absorbable components.

The Role of Digestive Enzymes in Nutrient Absorption

Digestion is a complex chemical process requiring lipase for fats, protease for proteins, and amylase for starches. When these are missing, the small intestine cannot process chyme. The resulting undigested material causes osmotic diarrhea, as the presence of food particles draws excess water into the bowel. This state is sometimes mismanaged as simple gastrointestinal sensitivity, but it is a distinct physiological failure that requires life-long medical support.

Clinical Manifestations of Malabsorption

Recognizing EPI involves observing subtle changes in a dog’s weight, stool quality, and overall energy levels. It is a slow, chronic process that often takes time to become apparent.

Classic Signs of Digestive Failure

  • Steatorrhea: The production of large, pale, greasy, or foul-smelling stools. This is a hallmark sign that dietary fats are passing through the system untouched.
  • Polyphagia: Despite eating large amounts of food, the dog fails to gain weight or continues to lose it. The body is essentially starving because it cannot access the calories ingested.
  • Weight Loss: Muscle wasting, or cachexia, is common as the dog consumes its own tissue for energy.
  • Coprophagia: Due to the intense hunger and nutrient deficiency, many dogs begin eating feces or other non-food items.

When to Seek Emergency Care

While EPI itself is chronic, complications can arise. Watch for signs of systemic collapse, lethargy, or extreme dehydration. Because the condition limits the body's ability to maintain homeostasis, it may worsen the prognosis of other issues like Chronic Hepatitis or metabolic shifts that resemble the secondary effects of Hyperadrenocorticism. If a dog stops eating or begins vomiting, seek immediate veterinary intervention to prevent severe metabolic crises.

Diagnostic Protocols for Pancreatic Function

Veterinarians diagnose EPI through specific serum biomarker analysis. The most reliable test is the Trypsin-Like Immunoreactivity (TLI) test. The primary treatment for EPI is life-long pancreatic enzyme replacement therapy (PERT). Powdered or encapsulated enzymes are added to every meal to replace the missing natural secretions. This allows the dog to absorb nutrients again, effectively halting weight loss and resolving diarrhea.

Strategic Nutritional Support

  • Enzyme Supplementation: High-quality porcine-derived enzymes are the standard. They must be mixed with food and, in some cases, allowed to sit for a few minutes to begin the predigestion process.
  • B12 Supplementation: Since the pancreas produces intrinsic factor, which is necessary for B12 absorption, most dogs with EPI require supplemental Vitamin B12 injections or oral medications for the remainder of their lives.
  • Dietary Adjustments: Some dogs benefit from a highly digestible, low-fiber, moderate-fat diet. However, strict fat restriction is no longer considered the universal gold standard, provided enough enzymes are administered.

EPI is often mistaken for inflammatory bowel disease or SIBO (Small Intestinal Bacterial Overgrowth).
