Feline Hepatic Lipidosis (FHL) occurs when the liver becomes infiltrated with excessive fat, hindering its ability to perform essential metabolic functions. This is not a primary disease, but rather a manifestation of severe protein-calorie malnutrition. When a cat enters a catabolic state, the mobilization of lipid stores from adipose tissue happens faster than the feline liver can metabolize or transport them out of the cells. The result is 'fatty liver,' which leads to cholestasis and severe hepatic dysfunction.

The Pathophysiology of Hepatocellular Triglyceride Accumulation

In the healthy feline liver, triglycerides are processed for energy or bundled into lipoproteins for export. During prolonged anorexia, the hormonal environment—characterized by low insulin and high glucagon levels—triggers the massive release of non-esterified fatty acids. The feline liver, which lacks some of the evolutionary efficiency found in other species for fat metabolism, is uniquely susceptible to this influx. The accumulated triglycerides literally displace the hepatocytes, causing intrahepatic cholestasis and ultimately leading to liver failure.

Clinical Manifestations and Diagnostic Indicators

Recognizing the warning signs early is critical for survival. The most classic sign is profound, rapid weight loss followed by sudden jaundice.

Identifying Early Warning Signs

  • Anorexia: Any period of fasting lasting more than 48 hours is a major red flag.
  • Icterus: Yellowing of the ears, gums, and third eyelids indicates significant bile duct blockage.
  • Ptyalism: Excessive drooling often results from the nausea associated with hepatic failure.
  • Lethargy: Profound muscle wasting and weakness are common as the cat enters a catabolic state.

If your cat shows signs of confusion or extreme lethargy, you must seek emergency care, as this may indicate the development of hepatic encephalopathy. When investigating potential primary causes, clinicians may also evaluate for comorbidities like Hyperadrenocorticism or endocrine imbalances that can mask or contribute to the anorexia.

Differential Diagnoses and Diagnostic Protocols

Because FHL is secondary to an underlying trigger, the diagnostic process must be thorough. Veterinarians often rule out Exocrine Pancreatic Insufficiency or other systemic conditions that may cause weight loss. In some cases, chronic kidney issues such as Glomerulonephritis or rare endocrine tumors like a Pheochromocytoma must be considered if routine diagnostics are inconclusive. Diagnostic imaging, specifically abdominal ultrasound, typically reveals a characteristic hyperechoic (bright) liver appearance, confirming the presence of excessive fat.

Management and Nutritional Support

Treatment relies entirely on aggressive, high-protein nutritional support. Because most affected cats are severely nauseated, force-feeding or syringe-feeding is often ineffective and stressful. The gold standard is the placement of a temporary feeding tube, such as a nasogastric or esophagostomy tube. This allows for controlled delivery of high-calorie liquid diets over several weeks, bypassing the cat's lack of appetite.

The Role of Supportive Pharmacotherapy

Beyond nutrition, treatment includes fluid therapy to correct electrolyte imbalances (potentially checking for Hypocalcemia or other mineral disturbances), anti-nausea medications, and liver-protectant supplements like S-adenosylmethionine (SAMe). It is also vital to monitor for potential vascular complications, such as Portal Hypertension, which can arise from structural changes in the damaged liver. If secondary hormonal disorders such as Acromegaly are discovered, they must be managed concurrently to achieve long-term remission.
