Progressive Retinal Atrophy (PRA) is a category of inherited disorders characterized by the bilateral, progressive degeneration of the retina. In dogs affected by PRA, these cells progressively deteriorate, eventually rendering the retina incapable of transmitting visual signals to the brain.

Early-Onset vs. Late-Onset Degeneration

There are two main variations of the condition. Early-onset forms, often referred to as retinal dysplasia or early-onset PRA, are sometimes seen in conjunction with other congenital issues like Collie Eye Anomaly. Late-onset forms, which may not manifest until the dog is several years old, are more common in breeds such as the Labrador Retriever and the Poodle. Unlike Multifocal Retinopathy, which involves specific focal lesions, PRA involves a global thinning of the retinal layers.

Owners often do not notice the onset of the disease because the degeneration is so gradual. The first sign is typically 'night blindness' (nyctalopia), where the dog becomes hesitant to go outside at night or navigate dimly lit rooms.

Indicators of Advanced Retinal Deterioration

  • Hesitancy in low-light environments.

  • Dilated pupils (mydriasis) that do not constrict under bright light.

  • Increased 'shine' or green/yellow reflection from the eyes in photographs.

  • Bumping into furniture or miscalculating jumps.

  • Sudden anxiety or increased reliance on the owner for navigation.

Diagnostic Protocols in Veterinary Ophthalmology

Diagnosis of PRA requires a comprehensive ocular examination. The veterinarian will typically use an ophthalmoscope to visualize the fundus, looking for classic signs of vascular attenuation and pigmentary changes in the tapetal and non-tapetal regions of the retina.

Advanced Diagnostic Modalities

For definitive diagnosis, a veterinary ophthalmologist may utilize Electroretinography (ERG), which measures the electrical activity of the photoreceptor cells. Additionally, DNA testing is now available for many breeds to confirm the presence of genetic mutations associated with specific variants of PRA, ensuring that conditions like Sphingomyelinase Deficiency are ruled out when neurological symptoms are absent.

Management Strategies for the Visually Impaired Canine

While no surgery or medication can restore sight once the cells have died, management focuses on environmental stability. Keeping furniture in the same positions, using scent markers for stairs, and avoiding sudden environmental changes can greatly assist a dog transitioning to total blindness. It is vital to note that PRA is distinct from conditions requiring systemic intervention, such as Factor VII Deficiency or Hemophilia B, as PRA remains localized to the ocular system.
