Understanding Reptile Metabolic Bone Disease (MBD)
Metabolic Bone Disease (MBD) represents a group of disorders involving the loss of skeletal mineralization. In reptiles, it is most frequently identified as nutritional secondary hyperparathyroidism. Because reptiles are ectotherms, their physiological processes are inextricably linked to their environmental inputs. If a lizard suffers from Iguana Hypocalcemia due to poor diet, the body enters a compensatory state where the parathyroid gland releases hormones to leach calcium from bones to keep the heart and nervous system functioning.
Primary Drivers of Skeletal Fragility

Calcium/Phosphorus Imbalance: Diets high in phosphorus interfere with calcium absorption.

UVB Deficit: Without natural or artificial full-spectrum light, the animal cannot synthesize Vitamin D3.

Vitamin D3 Mismanagement: Excessive or absent supplementation disrupts the endocrine feedback loop.
Symptomatology and Clinical Progression
Symptoms of MBD often progress insidiously. Owners may notice their pet slowing down, refusing food, or moving with an unusual gait.
Early Warning Signs

Advanced Physical Deformities

Skeletal Distortion: Permanent spinal kinks, kyphosis (hunchback), or bowed limbs that make locomotion painful.
Diagnostic Protocols in Exotic Medicine
Veterinarians approach MBD with a combination of clinical assessment and diagnostics. A physical examination typically reveals skeletal deformities and palpation of the jaw. Radiography (X-rays) is the gold standard for visualizing bone density loss and previous healing fractures. Blood chemistry panels are used to assess the calcium-to-phosphorus ratio and evaluate renal function, as the kidneys are critical in calcium regulation. In some cases, clinicians may assess Obesity as a comorbid factor, as fat deposits can sometimes mask early skeletal thinning.
Therapeutic Interventions and Supportive Care
Treatment must occur in phases. The first priority is stabilizing the blood calcium levels through calcium gluconate injections or oral supplementation. This is followed by the implementation of proper UVB lighting (often T5 HO bulbs replaced every 6 months) and a strict diet change. Surgical intervention may be necessary for severe fractures, though internal fixation is risky in reptiles with significantly depleted bones. If left untreated, symptoms can mimic other systemic issues, so it is vital to rule out secondary infections or Pancreatitis which may cause non-specific lethargy.
Prevention and Long-Term Management
Prevention is fundamentally about husbandry. Captive environments must replicate wild conditions as closely as possible. This includes high-quality UVB output, varied plant-based or insect-based diets with appropriate calcium dusting, and monitoring for Dietary Indiscretion or inappropriate substrate ingestion. While MBD is not inherently contagious, poor husbandry practices often affect entire collections simultaneously, leading to herd-wide health issues.
