Understanding Spinal Arachnoid Cysts and Subarachnoid Pathophysiology
A spinal arachnoid cyst, often referred to as a subarachnoid diverticulum, represents a localized focal dilation of the subarachnoid space. These fluid-filled structures occur when the arachnoid membrane—one of the three meningeal layers covering the central nervous system—becomes thickened and adheres to the spinal cord or the surrounding dura mater. Unlike Intervertebral Disc Disease, which involves the extrusion of disc material, an arachnoid cyst represents a space-occupying lesion derived from the body's own cerebrospinal fluid (CSF) dynamics.
Types of Subarachnoid Space Occupying Lesions

Congenital Cysts: Often developmental in nature, these are frequently observed in juvenile dogs, particularly those from predisposed breeds, suggesting a possible hereditary component to the structure of the meninges.

Acquired Cysts: These arise secondary to chronic inflammation, previous spinal trauma, or conditions like Discospondylitis. Inflammatory exudate can cause focal adhesions, effectively walling off sections of the subarachnoid space.
Clinical Manifestations and Neurological Deficits
The onset of symptoms in dogs with spinal arachnoid cysts is typically insidious. Because the cyst expands slowly, the spinal cord often accommodates the pressure to some degree before overt clinical signs manifest. However, once a critical threshold of compression is reached, the dog may show rapidly deteriorating mobility.
Early Warning Signs

Subtle stumbling or "knuckling" of the rear paws.

Muscle atrophy in the limbs due to disuse or nerve damage.

Reluctance to jump, climb stairs, or navigate slick surfaces.

Diagnostic Protocols: Localizing the Compression
Veterinarians must differentiate spinal arachnoid cysts from other space-occupying lesions such as a Spinal Cord Tumor. The diagnostic journey usually begins with a comprehensive neurological examination, during which the veterinarian assesses proprioception, reflex arcs, and nociception to localize the lesion.
Advanced Imaging Modalities
- Magnetic Resonance Imaging (MRI): The gold standard for identifying subarachnoid diverticula. MRI allows clinicians to visualize the cystic wall, the degree of spinal cord compression, and the presence of secondary syrinx (syringomyelia).
- Computed Tomography (CT) Myelography: While less sensitive than MRI, this can be utilized when MRI is unavailable to confirm the presence of an extramedullary space-occupying mass by visualizing the contrast column flow interruption.
- Cerebrospinal Fluid (CSF) Tap: Sometimes performed to rule out inflammatory or infectious myelitis, though it is rarely diagnostic for the cyst itself.
Surgical and Medical Management Strategies
Surgery is the primary treatment for symptomatic spinal arachnoid cysts. The objective is to perform a laminectomy to visualize the spinal cord and then perform a 'marsupialization' or fenestration of the cyst to allow the trapped CSF to drain and prevent future accumulation.
Post-Surgical Recovery and Rehabilitation
Recovery requires strict activity restriction for 6 to 8 weeks. Physical therapy, including range-of-motion exercises, hydrotherapy, and assisted ambulation, is critical for restoring motor function. In cases where surgery is not an option, medical management involving corticosteroids or anti-inflammatory drugs may be attempted to reduce edema, though this rarely offers a long-term solution for the structural compression.
