Aortic stenosis is a mechanical obstruction in the left ventricular outflow tract. In canine patients, it is most commonly congenital, often appearing as a subvalvular fibrous ring or tunnel-like narrowing beneath the aortic valve leaflets. Because the left ventricle must generate significantly higher pressures to force oxygenated blood through the narrowed aorta, the heart muscle undergoes remodeling. This process is distinct from conditions like Myocarditis, where inflammation causes damage to the heart tissue.
Types of Left Ventricular Outflow Obstruction

While Arrhythmogenic Right Ventricular Cardiomyopathy often involves right-sided electrical failures, aortic stenosis presents a unique risk for sudden ventricular arrhythmia during exercise, which can be fatal.
Genetic Predisposition and Diagnostic Imaging
Aortic stenosis has a strong hereditary basis, particularly in large and giant breeds. Unlike Patent Ductus Arteriosus, which is a failure of a fetal blood vessel to close, aortic stenosis is a failure of structural development of the outflow tract.
Diagnostic Tools for Cardiac Evaluation
- Auscultation: Identifying a characteristic left basilar murmur.
- Echocardiography: The gold standard for visualizing the subaortic ridge and measuring the pressure gradient using Doppler flow velocities.
- Electrocardiography (ECG): Monitoring for signs of Ventricular Tachycardia or other arrhythmias.
- Thoracic Radiography: Assessing for cardiomegaly or secondary signs of left heart failure, which must be distinguished from Pericardial Effusion or Mitral Stenosis.
Management Strategies and Long-term Care
Therapy focuses on symptom management and cardiac protection. Beta-blockers, such as atenolol, are the cornerstone of medical management. These medications lower the heart rate, providing more time for the left ventricle to fill with blood and reducing the myocardial oxygen demand of the hypertrophied muscle wall. Unlike Atrial Fibrillation, which requires rate and rhythm control, aortic stenosis management prioritizes preventing dynamic obstruction.
Long-term Prognosis and Lifestyle Restrictions
Dogs diagnosed with aortic stenosis should avoid strenuous exercise, intense training, or situations that induce high levels of excitement. This limits the risk of acute syncopal episodes. Regular cardiac check-ups are essential, as the severity of the stenosis can progress or the heart may eventually progress toward congestive heart failure. Owners must watch for subtle shifts in energy levels, which may indicate that the heart is no longer compensating adequately for the narrowed outflow tract.
