Mitral stenosis is a mechanical obstruction of the mitral valve, which acts as the gateway between the left atrium and left ventricle. Unlike Mitral Valve Disease, where the valve fails to close properly, stenosis involves the failure of the valve to open adequately. This creates a pressure gradient that the heart must overcome, leading to significant structural changes.

Hemodynamics and Pathophysiology

When the mitral valve opening is constricted, blood flow is impeded during diastole. This resistance forces the left atrium to generate higher pressures to maintain flow, resulting in atrial dilation. Because the left atrium is connected directly to the pulmonary veins, this back-pressure is transmitted to the lungs, often resulting in Congestive Heart Failure.

Clinical Manifestations and Respiratory Indicators

Detecting mitral stenosis in the early stages can be challenging, as dogs often compensate for the reduced flow for long periods.

Common Signs of Cardiac Inefficiency

  • Exercise intolerance during routine walks or play

  • Tachypnea (increased respiratory rate at rest)

  • Persistent, non-productive coughing

  • Syncope (fainting) due to decreased cardiac output

  • Weakness or lethargy following minor exertion

Emergency Warning Signs

If your dog experiences sudden respiratory distress, blue-tinted gums (cyanosis), or a collapse, this indicates an urgent need for medical intervention, as these are signs of acute Congestive Heart Failure.

Etiology: Congenital vs. Acquired Factors

In canine patients, mitral stenosis is most frequently a congenital defect, present from birth. It is often diagnosed alongside other congenital heart conditions like Patent Ductus Arteriosus. While rarer, acquired stenosis may occur following Myocarditis or endocarditis, where inflammation causes scarring and fibrosis of the valve leaflets.

Diagnostic Framework for Mitral Stenosis

Veterinarians typically suspect the condition upon hearing a diastolic Heart Murmur during a physical examination.

Imaging and Diagnostic Tools

  • Echocardiography: The gold standard for measuring the pressure gradient across the valve.

  • Thoracic Radiography: Used to visualize pulmonary congestion and left atrial enlargement.

  • Electrocardiogram (ECG): Essential for detecting arrhythmias such as Ventricular Tachycardia that may arise due to atrial stretching.

Management and Therapeutic Options

Management focuses on reducing the workload on the heart and controlling fluid accumulation. If a patient shows signs of fluid buildup, diuretics are utilized to relieve pressure. Medications to manage heart rhythm or improve contractility may also be prescribed. In extreme cases, surgical intervention, such as balloon valvuloplasty, may be considered by board-certified veterinary cardiologists, although this is complex and carries significant risks compared to treatments for Dilated Cardiomyopathy.
