Degloving injuries are dramatic and severe traumatic events. When skin is ripped away from the underlying muscle, bone, and connective tissue, the primary concern is the total loss of the blood supply. This results in the death of the detached skin, even if it looks attached at first glance. These injuries often present alongside a crush injury if the trauma resulted from a heavy weight or impact, complicating the treatment plan.

Types of Avulsion Injuries

  • Open Degloving: The skin is visibly torn, exposing subcutaneous tissue, fascia, or bone.

  • Closed Degloving (Morel-Lavallée lesion): The skin remains intact, but the connection between the skin and the deep fascia is severed, creating a large, fluid-filled pocket of blood and serum.

Clinical Manifestations and Emergency Indicators

Recognizing the severity of a traumatic wound is vital for the survival of the animal. While some owners may mistake a minor scrape for a degloving injury, true degloving represents a significant emergency. If you suspect an injury, look for signs of a pulmonary contusion if the trauma was a high-speed vehicle impact, as internal bleeding can occur alongside surface wounds.

Visible Warning Signs

  • Large flaps of skin dangling or appearing detached.

  • Areas where skin feels cold or lacks a normal color (blanching).

  • Excessive serosanguinous fluid accumulation under the skin.

  • Exposed bone or muscle structures without a protective skin layer.

Pathogenesis and Etiology

The root cause is high-velocity shearing force, most commonly seen in vehicular accidents, dragging, or entrapment. In some cases, pets may suffer a secondary electric shock if the injury occurred near power lines or infrastructure, which adds a layer of complexity to the metabolic stabilization of the patient.

Factors Influencing Prognosis

  • Depth of Tissue Loss: Full-thickness loss is significantly harder to manage than partial loss.

  • Vascular Integrity: The degree to which the perforating arteries are damaged determines how much skin can be salvaged.

  • Time to Debridement: Rapid intervention reduces the microbial burden and potential for full-thickness necrosis.

Veterinary Assessment and Diagnostic Protocols

Veterinarians approach these cases systematically. After stabilizing vital signs, the focus shifts to wound assessment. If the injury occurred in a high-risk area, such as the head, they must rule out a corneal ulcer (traumatic) caused by the same impact event.

Essential Diagnostic Procedures

  • Radiographic Evaluation: To check for associated fractures or joint luxation.

  • Physical Wound Mapping: Identifying the extent of the undermining using sterile probes.

  • Baseline Blood Panels: Monitoring for systemic inflammatory response syndrome (SIRS).

Surgical Reconstruction and Therapeutic Management

Surgery is almost always required. The process involves extensive surgical debridement of all non-viable tissue. Once the wound is cleaned, reconstructive techniques—ranging from tension-relieving incisions to skin grafts or flaps—are utilized to close the defect. Chronic wounds that fail to heal properly may also require management similar to an equine corneal ulcer in terms of frequent monitoring and topical support if the location allows.
