Joint luxation represents a complete loss of integrity in an anatomical joint, where the articular surfaces no longer maintain contact. This condition is most frequently a result of significant soft tissue trauma, such as vehicular collisions or high-impact falls. When a joint is luxated, the supporting fibrous structures—including the joint capsule and various ligaments—are torn, leading to extreme instability and acute pain.
Types of Luxation and Mechanical Presentation
Traumatic luxations are acute events that require immediate veterinary attention. In contrast, congenital or developmental luxations, often seen in specific breeds, occur due to anatomical malformations of the acetabulum or humeral head. Common sites for luxation include the coxofemoral (hip) joint, the scapulohumeral (shoulder) joint, and the cubital (elbow) joint. In some cases, trauma may be so severe that it presents alongside bone fractures or even spinal trauma in cases of high-impact incidents.

Identifying Acute Joint Instability
Owners typically notice a sudden onset of non-weight-bearing lameness. The limb may appear shortened or rotated at an abnormal angle. Swelling around the joint space is common as blood and synovial fluid extravasate into the surrounding tissue. In severe cases, particularly those involving the stifle, the presentation might be mistaken for a cruciate ligament rupture, requiring precise diagnostics to differentiate the two.
When to Seek Emergency Care
If the luxation is the result of a major trauma, the patient may suffer from systemic shock. Owners must watch for respiratory distress, which could indicate a pneumothorax or pulmonary contusion. Furthermore, in feline patients, falls from significant heights—often associated with high-rise syndrome—frequently result in complex luxations requiring multi-modal stabilization.
Diagnostic Protocols and Imaging
Veterinarians diagnose joint luxation through a combination of manual orthopedic palpation and diagnostic imaging. Manual manipulation often reveals a lack of the usual end-feel of the joint. Radiography is the gold standard for confirmation, as it allows the clinician to see the degree of displacement and rule out accompanying avulsion fractures. In cases where soft tissue damage is extensive or the luxation is chronic, advanced imaging such as CT scans may be used to plan surgical reconstruction.
Surgical and Conservative Management
Closed Reduction Techniques
In many fresh, uncomplicated cases, a veterinarian may attempt a closed reduction under heavy sedation. This involves manually guiding the joint back into its socket and potentially utilizing a supportive bandage, such as an Ehmer sling for hip luxations. While less invasive, closed reduction carries a higher risk of recurrence if the supporting ligaments are severely shredded.
Surgical Stabilization (Open Reduction)
If the joint is chronically luxated or fails to maintain its position after closed reduction, open reduction and internal fixation (ORIF) are necessary. Surgeons may use prosthetic ligaments, bone toggles, or even total joint replacement to ensure long-term stability and function. Post-operative care requires strict confinement to prevent premature stress on the surgical site.
