Equine Infectious Anemia (EIA), often referred to by horsemen as 'swamp fever,' is a severe viral disease caused by the equine infectious anemia virus (EIAV). It is a member of the Retroviridae family, specifically a lentivirus, which utilizes reverse transcriptase to integrate its genetic material into the host's cells. Unlike Equine Influenza or other common respiratory ailments, EIA is characterized by its ability to hide within the host's immune cells for their entire lifespan, periodically 'waking up' to cause systemic damage.
Clinical Manifestations: Acute, Subacute, and Chronic Phases
The disease cycle in EIA is notoriously unpredictable. In the acute phase, the horse experiences a rapid multiplication of the virus, leading to sudden fever, thrombocytopenia, and lethargy. As the virus mutates to evade the host immune system, the horse may undergo cycles of relapse, where symptoms reappear after periods of normalcy. These relapses are driven by the immune system's attempt to fight the virus, which ultimately results in the destruction of the horse's own red blood cells (hemolytic anemia).
Recognizing the Clinical Signs of EIA
Identifying EIA can be difficult because the symptoms often mimic other febrile conditions like Equine Viral Arteritis or severe systemic bacterial infections. Owners must be vigilant for subtle shifts in energy and health.

Transmission Dynamics and Risk Factors
Transmission occurs primarily through the transfer of blood from an infected horse to a susceptible one. The most common vectors are large biting flies, such as deer flies and horse flies, which carry blood on their mouthparts between hosts. Needles, dental tools, and other surgical equipment that are not properly sterilized represent an iatrogenic (human-caused) risk factor for spreading the virus throughout a barn.
Diagnostic Protocols: The Coggins Test and Beyond
The gold standard for diagnosing EIA is the Coggins test, which detects antibodies in the blood against the EIAV. This test, based on agar gel immunodiffusion (AGID), is required for interstate travel and show participation. In instances where a rapid result is required, Enzyme-Linked Immunosorbent Assay (ELISA) tests are utilized. A positive result on a screening test is always confirmed by a secondary, more specific laboratory assay to ensure no false positives exist, a practice similar to diagnostic rigor used for Bovine Viral Diarrhea (BVD) in the cattle industry.
Management, Regulation, and Legal Compliance
Because there is no cure, regulatory agencies mandate the permanent isolation or euthanasia of infected animals. Supportive care, such as fluid therapy or anti-inflammatory drugs, may be provided in private settings to ease clinical symptoms, but the animal remains infectious for life. If a veterinarian identifies a positive horse, the owner is legally required to comply with state and federal quarantine guidelines to protect the regional equine population. Unlike conditions like Caprine Arthritis Encephalitis (CAE), where specific management can sometimes contain the spread, EIA is considered too dangerous for anything less than total isolation or euthanasia.
