Understanding Feline Herpesvirus-1 (FHV-1) Pathogenesis
Feline Herpesvirus-1 (FHV-1) is a ubiquitous pathogen that represents the primary agent of feline viral rhinotracheitis. Unlike some acute illnesses that a cat may recover from fully, such as Canine Parvovirus, FHV-1 establishes a permanent residence within the host. The virus is a member of the *Herpesviridae
- family and is noted for its ability to go dormant in the nervous system, periodically reappearing when the immune system is compromised.
Viral Latency and Reactivation
After the initial acute infection, most cats become lifelong carriers. The virus remains dormant in the trigeminal ganglia. Stressors—such as moving house, the introduction of a new pet, boarding, or secondary illnesses—can trigger viral reactivation. It is during these periods of recrudescence that the cat becomes contagious again, shedding the virus in nasal and ocular fluids.
Clinical Manifestations and Ocular Complications
Clinical signs of FHV-1 are primarily respiratory and ocular in nature. It is often compared to Feline Calicivirus due to the overlapping respiratory symptoms, though FHV-1 is distinctly more likely to cause severe ocular pathology.
Ocular Pathologies

Conjunctivitis: Extreme redness and swelling of the tissues surrounding the eyes.

Dendritic Keratitis: The hallmark lesion of herpesvirus, appearing as branching ulcers on the surface of the cornea.

Respiratory Distress Indicators
Cats may exhibit frequent sneezing, nasal discharge, and lethargy. In severe cases, the nasal passages become so inflamed that the cat loses its sense of smell, leading to anorexia. It is critical to differentiate these signs from Feline Leukemia Virus (FeLV), which weakens the overall immune system and makes the cat more susceptible to persistent respiratory issues.
Diagnostic Protocols
Veterinarians typically diagnose FHV-1 based on clinical history and presentation. When symptoms are ambiguous, Polymerase Chain Reaction (PCR) testing is utilized to detect viral DNA. However, because many healthy cats carry the virus, a positive PCR result must be interpreted alongside current clinical signs. In cases involving chronic eye issues, a fluorescein stain is mandatory to assess for the presence of corneal ulcerations, which can be masked by the severe inflammation associated with this condition.
Management and Therapeutic Strategies
There is no cure for FHV-1. Treatment is supportive and aimed at minimizing clinical duration and intensity.

L-Lysine: An amino acid supplement often used to interfere with viral replication, though evidence regarding its efficacy remains debated.

Topical Antivirals: Medications like idoxuridine or cidofovir are frequently prescribed for active ocular outbreaks.

Antibiotic Coverage: If secondary bacterial infections (such as those causing an Otitis Externa (Ear Infection)) arise, antibiotics are necessary to manage the secondary load.
Long-Term Preventive Care
Vaccination is the cornerstone of prevention. While the FVRCP vaccine does not provide 100% immunity, it significantly reduces the severity of the clinical disease. Owners should focus on minimizing environmental stressors to prevent viral reactivation. For cats with chronic eye lesions, lifelong monitoring by a veterinary ophthalmologist is often the standard of care to preserve vision.
