Recurrent Airway Obstruction (RAO) is a chronic, debilitating disease that primarily impacts the small airways (bronchioles) of the equine lung. Unlike infectious conditions like pneumonia, RAO is an immunologically mediated hypersensitivity reaction. When a susceptible horse inhales environmental allergens, the immune system overreacts, leading to inflammation, bronchospasm, and excessive mucus secretion that clogs the delicate air passages.

Chronic inflammation in RAO leads to what veterinarians describe as bronchial remodeling. Over time, the smooth muscles surrounding the bronchioles undergo hypertrophy, and the airway walls thicken with fibrotic tissue. Horses with RAO may exhibit a flared nostril posture or a distinct heave line along the flank. In severe cases, the horse may lose weight, appear depressed, and exhibit extreme distress during high-heat or high-humidity days, which can sometimes be exacerbated by underlying Equine Metabolic Syndrome if the horse has become sedentary due to its breathing capacity.
Recognizing Acute Exacerbation (Emergency)
An acute episode of RAO is a medical emergency. Signs include:
- Extreme respiratory effort at rest (nasal flaring, abdominal pumping).

- Cyanosis (blue-tinged mucous membranes).
- Total refusal of feed or water.
- Panic or extreme lethargy.

Veterinarians utilize several tools to differentiate RAO from other pathologies like Inflammatory Bowel Disease (which can present with similar weight loss signs) or Chronic Kidney Disease (which may be a confounding systemic issue). Diagnostic procedures include:
- Bronchoalveolar Lavage (BAL): The gold standard for diagnosis, involving the infusion and collection of sterile saline from the lungs to analyze inflammatory cells like neutrophils.
- Endoscopy: Visualization of the trachea and bronchi to identify excessive mucus and inflammation.
- Pulmonary Function Testing: Assessing airway resistance in specialized clinical settings.
Therapeutic Management Strategies
Treatment for RAO is bifurcated into immediate symptom relief and long-term environmental modification. Systemic or inhaled bronchodilators, such as clenbuterol or albuterol, are frequently utilized to ease acute bronchoconstriction. Furthermore, inhaled corticosteroids are the standard for managing the inflammatory component of the disease while minimizing the systemic side effects seen with oral prednisone. Managing these horses is often more complex if they also require monitoring for endocrine disorders like Cushing's Syndrome.
Environmental Stewardship and Long-Term Care
Environmental management is the cornerstone of preventing recurrence. Simply put, if you remove the source of the allergen, you control the disease. This means transitioning to dust-free feeds, steaming hay, replacing straw bedding with dust-free shavings or paper, and ensuring maximum stall ventilation or transitioning to 24/7 pasture turnout. While some horses may appear to have concurrent metabolic issues, one must avoid the pitfalls of misdiagnosis, ensuring it is not actually Hypothyroidism causing the lethargy, though this is rare in horses.
