Feline Odontoclastic Resorptive Lesions (FORLs) are a pervasive dental health concern affecting a significant portion of the domestic cat population. Unlike a tooth fracture, which is often the result of sudden trauma, FORLs are a chronic, progressive process driven by the body's own cells.

Pathophysiology and Dental Erosion

At their core, FORLs involve the activation of odontoclasts—specialized cells that normally facilitate the resorption of baby teeth in kittens. When these cells activate inappropriately in adult cats, they begin to digest the mineralized tissues of the permanent teeth. This process typically begins at the neck of the tooth, right at the gum line, but can spread rapidly into the root structure.

Why Clinical Presentation Is Often Delayed

Cats are stoic animals that often hide signs of oral discomfort. Even with severely painful lesions that expose the pulp, many cats will continue to eat, though they may show signs like dropping food or preferring softer textures. A dental malocclusion can sometimes exacerbate the discomfort if the erupting or shifting teeth press against the lesions.

Recognizing Clinical Signs and Behavioral Changes

It is vital to distinguish between normal feline behavior and indicators of oral pain. Unlike an oral foreign body, which causes acute and sudden distress, FORL pain is often chronic.

Behavioral Indicators

  • Excessive drooling or ptyalism.

  • Shaking the head while eating.

  • Behavioral shifts, including aggression or social withdrawal.

  • Avoidance of hard food kibble.

Physical Findings

  • Red, inflamed, or bleeding gums (gingivitis).

  • Presence of hyperplastic (overgrown) gingival tissue covering the tooth.

  • Visible defects at the gumline or discoloration.

  • Associated signs of discomfort that might be misidentified as tonsillitis.

Diagnostic Protocols in Veterinary Dentistry

Because most resorption occurs beneath the gumline, a physical examination is insufficient for a complete diagnosis. Your veterinarian will utilize a comprehensive approach to determine the extent of the damage.

The Importance of Full-Mouth Radiographs

Full-mouth dental radiographs are the gold standard for diagnosing FORLs. These images reveal the extent of bone loss and whether the root is fusing to the jawbone (ankylosis) or disappearing (replacement resorption). Without these images, the veterinary team cannot determine if a full extraction or a crown amputation is appropriate.

Clinical Probing

Under general anesthesia, the dentist uses a specialized dental probe to check for "catches" or "dips" along the tooth neck. Even if the enamel appears intact, the probe can detect subtle resorptive activity.

Therapeutic Interventions and Surgical Management

Unfortunately, there is no way to reverse or stop the progression of FORLs. Once the odontoclasts are triggered, the tooth is typically considered non-viable.

Extraction vs. Crown Amputation

  • Full Extraction: This involves the complete removal of the crown and all root structures. It is preferred when the periodontal ligament is healthy.

  • Crown Amputation: In cases where the root is completely resorbed and fused to the bone, removing the entire root may cause unnecessary trauma to the jaw. In these cases, the crown is surgically removed and the gingiva is sutured over the remaining root fragments.

Preventing Oral Complications

While you cannot strictly "prevent" the development of FORLs, you can maintain oral health to ensure that other conditions do not complicate the diagnosis, such as avoiding orodental trauma or managing chronic oral inflammation.

Home Care Routine

  • Daily brushing with cat-specific toothpaste.

  • Feeding dental-approved diets that help reduce plaque buildup.

  • Annual veterinary dental cleanings to allow for professional monitoring of the gingival health.
