Understanding Canine Laryngeal Paralysis
Laryngeal paralysis is a condition where the arytenoid cartilages in the larynx fail to abduct (open) during inspiration. Under normal circumstances, the cricoarytenoid dorsalis muscles pull these cartilages wide to allow air to pass into the trachea. When the recurrent laryngeal nerve—a branch of the vagus nerve—fails, these muscles atrophy, and the cartilages remain in a fixed, closed position. This creates a significant bottleneck for airflow, resulting in the characteristic sounds and respiratory struggle associated with the condition.
Idiopathic vs. Secondary Neuropathies
Most canine cases are classified as idiopathic, meaning the specific cause is unknown, yet research links these to generalized polyneuropathy. Sometimes, this can mimic symptoms of Wobbler Syndrome due to the presence of systemic nerve issues. Rarely, the condition is secondary to trauma, neoplasia, or endocrine diseases like hypothyroidism.
Clinical Manifestations and Respiratory Symptoms
The Warning Signs of Airway Obstruction
Symptoms often manifest gradually, starting with a change in the voice. Owners frequently notice the dog's bark becoming hoarse or 'weak.' As the opening narrows, the following symptoms arise:
- Stridor: A high-pitched, rasping sound during inhalation.
- Exercise intolerance: A reluctance to engage in physical activity due to low blood oxygen.
- Dyspnea: Visible effort required to expand the chest for air.
- Hyperthermia: Because dogs pant to cool down, an airway obstruction makes heat stroke a significant risk.
- Coughing: Often occurring after eating or drinking due to aspiration.
Diagnostic Protocols in Veterinary Medicine
Laryngoscopy and Neurological Evaluation
Diagnosis is confirmed via a sedated laryngeal exam. A veterinarian will use a laryngoscope to observe the movement of the arytenoids while the dog is lightly anesthetized. If the cartilages do not abduct in sync with respiration, the diagnosis is confirmed. Veterinarians must also rule out other neurological conditions like Meningitis or Botulism if the patient shows signs of widespread nerve failure.
Surgical Intervention and Management
Unilateral Arytenoid Lateralization
The most effective treatment is the 'tie-back' surgery, or unilateral arytenoid lateralization. This procedure uses non-absorbable sutures to permanently pull one of the arytenoid cartilages to an open position. While this effectively clears the airway, it permanently disables the larynx's ability to close, meaning the dog must be carefully managed to prevent food or water from entering the lungs. Post-operative care includes strictly controlled activity and the use of elevated feeding stations.
