Miliary dermatitis is a hallmark feline skin reaction pattern that feels like small, granular bumps—resembling millet seeds—when petting the cat. This is frequently linked to folliculitis, where the hair follicles become inflamed. This reaction is the feline's 'go-to' response for a variety of allergens, particularly flea saliva.
The Role of Hypersensitivity
Most cases are triggered by flea bite hypersensitivity. Even a single bite can trigger an immune-mediated cascade. However, other triggers like food allergies, contact dermatitis, and environmental sensitivities are common. If left untreated, the inflammation may progress to severe lesions.
Recognizing Clinical Presentation and Warning Signs
Identifying the condition requires a careful physical examination. Owners should watch for small, red, raised papules topped with dark crusts.

Because miliary dermatitis is a response, vets must rule out other conditions such as pemphigus foliaceus or systemic lupus erythematosus, which are autoimmune in nature. Other differentials include psychogenic grooming, feline paraneoplastic alopecia, or even chronic solar dermatitis in light-colored cats exposed to UV light. Sometimes, symptoms are confused with indolent (rodent) ulcer if facial involvement is present.
Diagnostic Protocols and Clinical Testing
Veterinarians use a systematic approach to confirm the trigger:
- Skin Cytology: To identify bacterial or yeast overgrowth.
- Skin Scrapings/Trichogram: To rule out parasites like *Cheyletiella
- or Demodex.

- Flea Prevention Audit: Verification of year-round parasite control.
Long-Term Management and Supportive Care
Treatment is centered on the underlying cause. If fleas are the culprit, aggressive environmental control is necessary. For environmental allergies, corticosteroids or newer medications like Oclacitinib may be used to suppress the itch signal. Topical antimicrobial therapy is often used to manage secondary infections. Long-term management involves keeping the cat on strict parasite preventative medication and potentially long-term dietary modification.
