Understanding Ectopic Ureters and Urogenital Anatomy

Ectopic ureters represent a developmental failure where the distal ureters do not insert into the bladder trigone. This anatomy is inherently flawed because the bladder, which is designed to store urine at low pressure, is bypassed, resulting in urine dribbling that the dog cannot control.

Extramural vs. Intramural Ureteral Pathways

In an extramural ectopic ureter, the ureter bypasses the bladder entirely, inserting directly into the urethra or reproductive tract. Conversely, an intramural ectopic ureter tunnels beneath the bladder mucosa before opening into the urethra. The latter type is the most common presentation in canine patients, often allowing for minimally invasive intervention.

Clinical Presentation and Diagnostic Indicators

The hallmark sign is persistent urine dribbling. Because the sphincter mechanism of the bladder neck is bypassed, the dog will leak urine even while sleeping or playing. Owners may notice the odor of ammonia or consistent wetness around the hindquarters. While some dogs may be diagnosed with Cryptorchidism concurrently, the primary focus remains the renal health of the patient.

Complications of Chronic Incontinence

  • Recurrent bacterial cystitis due to retrograde flow.

  • Perivulvar scald and secondary bacterial dermatitis.

  • Hydroureter and hydronephrosis due to secondary urine obstruction.

Diagnostic Procedures and Imaging Protocols

Veterinarians use a combination of techniques to confirm the diagnosis. A standard physical exam is rarely sufficient. Instead, contrast-enhanced radiography, excretory urography, or CT excretory urography (CTEU) are the gold standards to map the abnormal termination point.

Cystoscopic Evaluation

Cystoscopy is the definitive diagnostic and often therapeutic tool. It allows the surgeon to visualize the interior of the bladder and urethra directly to identify the misplaced ureteral opening. Unlike Tricuspid Valve Dysplasia, which is a cardiovascular defect, ectopic ureters require specifically genitourinary visualization.

Treatment Modalities: Surgical vs. Endoscopic

The goal of treatment is to move the ureteral opening into the bladder trigone.

Laser Ablation (Cystoscopic)

For intramural ectopic ureters, a laser can be used to incise the tunnel, creating a new opening inside the bladder. This is less invasive than traditional surgery.

Surgical Reimplantation

In cases where laser ablation is not feasible, an open surgical approach is required to reposition the ureter. This is a complex procedure often performed by board-certified veterinary surgeons.

Post-Operative Management and Long-Term Outlook

Success rates vary depending on the severity of the ectopic ureters and whether the dog has associated internal urethral sphincter incompetence. Some dogs require long-term management with medications like phenylpropanolamine or estrogen analogs to maintain continence. Regular monitoring for Pulmonic Stenosis or other systemic congenital issues is not standard unless clinical signs dictate, but ongoing urinalysis is essential to monitor for urinary tract infections.
