Feline Infectious Peritonitis (FIP) is an immune-mediated disease triggered by a mutation of the feline enteric coronavirus. While the enteric virus is ubiquitous in feline populations, FIP occurs only when the virus undergoes a mutation that alters its biological behavior. This mutation transforms the virus into one capable of infecting macrophages, white blood cells that then carry the virus throughout the body, inducing severe, systemic inflammatory responses.

The Distinction Between Effusive and Non-Effusive FIP

The clinical presentation of FIP is categorized into two main types. Effusive (wet) FIP is characterized by vascular damage, leading to the leakage of protein-rich fluid into the abdominal or pleural spaces. This is the more rapidly progressive form. Non-effusive (dry) FIP is characterized by the formation of pyogranulomas—clumps of immune cells—within organs such as the liver, kidneys, and central nervous system. Cats with the dry form may show more chronic, non-specific signs of illness. Interestingly, some cats may present with comorbid conditions, and veterinarians must be careful to rule out immunosuppressive illnesses like Feline Leukemia Virus (FeLV), which can complicate the prognosis.

Clinical Manifestations and Diagnostic Workup

Recognizing the clinical signs of FIP early is vital for treatment success. Symptoms range from vague, malaise-like behaviors to acute organ failure. In many cases, owners notice a gradual decline before the onset of more severe clinical indicators.

Systemic Symptoms and Emergency Indicators

  • Progressive weight loss and anorexia

  • Fluctuating fevers that do not respond to antibiotics

  • Abdominal distention caused by fluid accumulation (ascites)

  • Respiratory difficulty or dyspnea from thoracic effusions

  • Ocular changes such as uveitis, hemorrhage, or cloudiness

  • Neurological symptoms including ataxia, seizures, or tremors

Laboratory and Imaging Protocols

Diagnosis is rarely based on a single test. Veterinarians typically utilize a combination of physical examination, complete blood counts (CBC), serum biochemistry, and fluid analysis. If fluid is present, laboratory analysis (rivalta test or protein-to-A/G ratio) is highly suggestive of FIP. Ultrasound is essential for identifying organ granulomas or localizing effusions. It is important to note that while Feline Calicivirus and other respiratory pathogens can cause ocular or oral signs, they do not produce the systemic granulomatous lesions associated with FIP.

Therapeutic Interventions and Management Strategies

For decades, FIP had no curative treatment. Current standard-of-care, while still evolving, revolves around potent antiviral medications that inhibit viral replication. These treatments are intensive and require dedicated monitoring by a veterinarian.

Pharmacological Management

The gold standard currently involves the use of nucleoside analogs. These drugs must be administered consistently for at least 12 weeks. Supportive care is also critical; this includes hydration, nutritional support, and immunomodulatory agents if appropriate. In cases where the nervous system is involved, the treatment regimen may need to be adjusted to ensure sufficient medication levels reach the brain and spinal cord.

Prevention and Long-Term Outlook

Preventing FIP is difficult because the primary culprit, the feline enteric coronavirus, is highly contagious and widespread. Most cats carry the virus without ever developing FIP. Prevention focuses on minimizing stress in multi-cat households and ensuring optimal hygiene to reduce the viral load in the environment. While there is no widely accepted, highly effective vaccine, maintaining the general health of the cat is the best line of defense. As Rabies and other common infectious diseases require strict vaccination protocols, owners should ensure that overall immune health is bolstered through comprehensive veterinary care and proper nutrition.
