Understanding Turbulent Blood Flow and Cardiac Auscultation

A heart murmur is an additional sound detected during a cardiac exam, created by the disruption of smooth laminar blood flow. When blood flow becomes turbulent, it creates vibrations detectable by a stethoscope. These sounds occur during systole (contraction) or diastole (relaxation). Importantly, a murmur is a sign, not a diagnosis. To determine the significance, veterinarians perform an echocardiogram to visualize cardiac chambers and valve function.

Grading Murmur Intensity

Veterinarians classify murmurs on a scale of I to VI. A Grade I murmur is extremely faint, while a Grade VI is loud enough to be heard even when the stethoscope is lifted slightly off the chest wall. Interestingly, the intensity of the murmur does not always correlate with the severity of the underlying heart disease.

Etiology: Structural vs. Physiological Origins

Many murmurs in young puppies are 'innocent' and disappear as the animal matures. In adults, however, murmurs are usually indicative of structural changes such as Mitral Valve Disease, where the valves fail to close properly, leading to regurgitation. In larger breeds, we must consider Dilated Cardiomyopathy, while cats may present with Restrictive Cardiomyopathy.

Acquired Heart Disease Factors

Acquired conditions often develop as a secondary effect of chronic issues, such as Systemic Hypertension. Additionally, parasitic infections like Dirofilaria immitis (Heartworm) can obstruct blood flow, resulting in an audible murmur. The presence of Pulmonary Hypertension may also contribute to increased cardiac strain.

Identifying Clinical Warning Signs

While many pets with a murmur appear entirely normal, owners should monitor for signs of Congestive Heart Failure.

Symptoms of Impending Decompensation

  • Persistent coughing or wheezing

  • Exercise intolerance

  • Lethargy or unusual weakness

  • Syncope (Fainting)

  • Sudden respiratory distress (open-mouth breathing in cats)

Managing Secondary Complications

Should cardiac function decline, pets may be at risk for Thromboembolism (Saddle Thrombus) or complex electrical issues like Atrioventricular Block. Always consult your veterinarian if you notice a change in your pet's breathing pattern or energy level.

Diagnostic Pathways in Cardiology

Diagnostics typically begin with a physical exam, followed by thoracic radiographs to assess heart size and lung fields. An echocardiogram remains the 'gold standard' for visualizing the valves, measuring chamber thickness, and calculating fractional shortening. In some cases, an EKG is required to monitor for arrhythmias that often accompany heart murmurs.

Therapeutic Strategies

Treatment is entirely dependent on the root cause. If the murmur is associated with a specific disease, medications such as ACE inhibitors, pimobendan, or diuretics are prescribed to manage workload and fluid retention. There is currently no surgical 'cure' for most murmurs, though valve repair techniques are emerging in specialized veterinary cardiology centers.
