Understanding Multiple Myeloma in Dogs

Multiple myeloma is a sophisticated oncological condition arising from the abnormal proliferation of plasma cells, a type of white blood cell responsible for antibody production. While similar in some clinical manifestations to Soft Tissue Sarcoma, multiple myeloma is specifically a disease of the hematopoietic system. It is distinct from solitary Plasma Cell Tumor because it involves the bone marrow as the primary site of origin and often results in systemic, multi-organ effects.

The Mechanisms of Malignancy: Hyperviscosity and Osteolysis

The malignancy causes two primary systemic crises: hyperviscosity syndrome and skeletal bone loss. The overproduction of immunoglobulin proteins makes the blood thick and viscous, forcing the heart to work harder and reducing perfusion to sensitive micro-vascular beds like the retina and the brain. Simultaneously, the myeloma cells signal the body to destroy bone tissue, leading to lytic lesions that appear on radiographs. This is quite different from the rapid, localized tissue invasion seen in Histiocytic Sarcoma.

Clinical Indicators of Myeloma Progression

Recognizing the disease early is difficult due to the non-specific nature of early symptoms. Owners should be vigilant for signs of lethargy, exercise intolerance, and unexplained lameness. As the disease advances, complications such as blindness or seizures may occur due to hyperviscosity, which may be confused with symptoms of Brain Tumor (Glioma). Excessive urination and thirst occur as a result of calcium imbalances in the kidneys.

Emergency Red Flags

  • Sudden onset blindness (retinal hemorrhage)

  • Acute collapse or neurological deficits

  • Non-weight bearing lameness (possible fracture)

  • Severe lethargy and refusal to eat

Diagnostic Approaches to Hematologic Malignancy

Diagnosis involves a multi-modal approach. Veterinarians often begin with a complete blood count and serum chemistry panel, which may show elevated globulins and calcium levels. A serum protein electrophoresis is the gold standard for identifying the monoclonal gammopathy characteristic of this condition. Radiographic imaging is required to visualize bone lysis, which helps distinguish it from other masses like Hemangiosarcoma or Transitional Cell Carcinoma that may affect other systems.

Advanced Testing

  • Bone marrow aspiration and biopsy

  • Radiographic skeletal survey

  • Urinalysis (to detect Bence-Jones proteins)

Therapeutic Strategies and Management

Treatment focuses on chemotherapy, typically using alkylating agents like melphalan and corticosteroids like prednisone. These drugs aim to suppress the cancerous clones and lower the production of M-proteins. In cases where hypercalcemia is present, aggressive fluid therapy and bisphosphonates may be used to protect the kidneys and strengthen bones. While this treatment is intense, it is far more managed than the surgical approach often taken with a Gastrointestinal Stromal Tumor.

Long-Term Monitoring

Management is lifelong, requiring regular blood work to monitor immunoglobulin levels and renal function. Unlike Ceruminous Gland Adenocarcinoma, which is often treated surgically, multiple myeloma is managed as a chronic, albeit progressive, disease.
