Understanding Heartworm-Associated Respiratory Disease (HARD) in Felines

Heartworm-Associated Respiratory Disease is fundamentally an inflammatory response to Dirofilaria immitis (Heartworm) within the feline pulmonary vasculature. When larvae reach the pulmonary arteries, they cause significant damage even before they mature into adults. This results in obstructive vascular lesions and, subsequently, Pulmonary Hypertension due to the narrowing of blood vessels.

Vascular Inflammation and Airway Hypersensitivity

The immune system reacts to these parasites by releasing inflammatory mediators, which induce bronchial smooth muscle constriction. This mimics Restrictive Cardiomyopathy in its severity, as the lung tissue becomes stiff and less compliant. Cats may experience severe dyspnea, which can be mistaken for other underlying cardiac issues like Unclassified Cardiomyopathy.

Clinical Manifestations and Diagnostic Recognition

Identifying HARD requires a high index of suspicion, as symptoms are highly variable and frequently intermittent. Chronic cough is the most common early indicator, often misdiagnosed as feline asthma.

When Respiratory Signs Become Urgent

If a cat suddenly exhibits open-mouth breathing, lethargy, or extreme exercise intolerance, it may signal an acute complication such as a Thromboembolism (Saddle Thrombus). Cats with severe vascular damage may also demonstrate signs similar to Atrioventricular Block if the cardiac output is compromised by extreme pulmonary pressure.

Advanced Diagnostic Modalities

Veterinarians typically use a combination of thoracic radiography to look for interstitial patterns and echocardiography to assess the pulmonary artery diameter. While antigen tests are specific, they are often negative in cats with immature infections, making antibody testing a more sensitive screen for exposure. In cases where the heart structure is significantly affected, ruling out Pericardial Effusion is necessary during the cardiac workup.

Therapeutic Strategies and Management

Treatment is predominantly supportive as there is no safe approved adulticide for cats. Management focuses on reducing the intense pulmonary inflammation using corticosteroids and managing potential secondary heart failure or obstructive issues. Long-term management requires strict indoor-only status and monthly preventative administration to prevent further larval arrival.
