Understanding Left Ventricular Myocardial Hypertrophy
Hypertrophic cardiomyopathy (HCM) is a primary disease of the feline heart characterized by the concentric thickening of the ventricular walls. Unlike Dilated Cardiomyopathy, where the heart muscle thins and dilates, HCM causes the muscle to become stiff and bulky, reducing the space for blood to enter. This reduction in volume and compliance forces the heart to work harder, eventually leading to secondary issues.
Genetic Basis and Breed Predisposition
Many cases are inherited, particularly in breeds like Maine Coons and Ragdolls, which possess known genetic mutations affecting myosin-binding protein C. However, it can manifest in any breed and frequently occurs in Domestic Shorthairs as an idiopathic or acquired condition associated with Systemic Hypertension or hyperthyroidism.
Clinical Manifestations and Respiratory Compromise
Recognizing the subtle signs of cardiac strain is critical for owners. While some cats are identified via a routine heart murmur heard by a veterinarian, others present with acute crises.
Signs of Impending Heart Failure

The Risk of Arterial Thromboembolism
A major concern with feline HCM is the stagnation of blood in the left atrium, which can result in clots. When these travel, they lead to Thromboembolism (Saddle Thrombus), a severe condition causing cold, blue-padded hind limbs and acute lameness. This is a medical emergency.
Diagnostic Protocols and Imaging
Veterinary diagnosis involves a multi-modal approach to differentiate HCM from Restrictive Cardiomyopathy or Unclassified Cardiomyopathy.
Echocardiographic Assessment
Echocardiography is the gold standard for visualizing myocardial thickness and valve functionality. Veterinarians assess the heart’s diastolic function, look for secondary mitral valve displacement, and calculate the Left Atrial Enlargement index.
Biochemical and Biomarker Testing

NT-proBNP: A blood test that indicates cardiac wall stretch.

Thyroid panel: Crucial to rule out thyrotoxicosis-induced hypertrophy.

Blood pressure monitoring: Necessary to rule out systemic hypertension as the underlying driver of myocardial thickening.
Therapeutic Strategies for Long-term Management
Treatment focuses on improving diastolic filling and preventing thrombus formation. Beta-blockers, such as atenolol, may be used to lower the heart rate and reduce myocardial oxygen demand. ACE inhibitors or calcium channel blockers are often incorporated to manage vascular resistance. If Congestive Heart Failure is present, diuretics like furosemide are essential to remove pulmonary fluid. Clopidogrel is the standard of care for preventing arterial clots in high-risk patients with significant left atrial enlargement.
