Systemic hypertension is a condition characterized by high blood pressure in the arterial circulation. In cats, this is rarely an isolated 'disease' of aging but rather a systemic manifestation of underlying pathology. The damage resulting from high pressure is referred to as Target Organ Damage (TOD), which predominantly affects the ocular, neurological, cardiac, and renal systems. Because cats often show no symptoms until the pressure is severely elevated, this condition is frequently described as a 'silent killer.'
Ocular Complications and Retinal Detachment
The eyes are perhaps the most sensitive organs to changes in systemic pressure. Elevated pressure causes the blood vessels in the retina to become leaky or rupture, leading to subretinal hemorrhage. If the pressure remains high, the retina may detach entirely, which is a common cause of sudden blindness in middle-aged and senior cats. Owners may observe fixed, dilated pupils that do not constrict in bright light as a primary warning sign.
Neurological and Central Nervous System Involvement
When pressure exceeds the autoregulatory capacity of the brain, hypertensive encephalopathy can occur. This manifests as behavioral changes, disorientation, seizures, or even an acute stroke-like episode. This is often confused with other neurological issues or secondary effects of restrictive cardiomyopathy, which can complicate the clinical picture.
Diagnostic Protocols for Elevated Arterial Pressure
Doppler Sphygmomanometry and High-Frequency Ultrasonography
Veterinarians diagnose hypertension using specialized equipment, usually a Doppler flow probe or an oscillometric device. Multiple measurements are required in a quiet environment to rule out 'white coat hypertension'—a transient spike in blood pressure caused by the stress of being in a veterinary clinic. Once confirmed, diagnostic imaging such as an echocardiogram may be used to assess the heart's size and structure, potentially uncovering conditions like unclassified cardiomyopathy.
Laboratory Screening for Systemic Disease
To determine the cause, a full chemistry panel, complete blood count, and total T4 (thyroid hormone) test are mandatory. Hypertension is often a 'telltale' sign that the patient is suffering from chronic kidney disease or hyperthyroidism, both of which require distinct, long-term management strategies.

Amlodipine Besylate as First-Line Treatment
Amlodipine is the gold standard calcium channel blocker used to reduce systemic pressure in cats. It works by relaxing the smooth muscles of the arterial walls, effectively lowering resistance to blood flow. Regular monitoring of the patient's blood pressure is crucial during the first two weeks of treatment to prevent hypotension.
Management of Concurrent Chronic Conditions
Treating the underlying disease is equally as important as controlling the pressure. If hyperthyroidism is identified, radioactive iodine or methimazole therapy must be implemented alongside blood pressure meds. For cats showing signs of restrictive cardiomyopathy, the treatment regimen must be carefully balanced so that it does not exacerbate existing heart failure symptoms.
Emergency Indicators and When to Seek Help
If a cat exhibits sudden blindness, acute ataxia, or altered mental status, this is an emergency. These signs often point to severe internal hemorrhage or neurological crisis. While not always directly linked, owners should monitor for signs of thromboembolism (saddle thrombus), which can be a secondary complication of cardiovascular strain caused by persistent hypertension. Immediate veterinary intervention is necessary to prevent permanent neurological or ocular loss.
