Feline Aortic Thromboembolism, frequently referred to as a 'saddle thrombus,' occurs when a blood clot (thrombus) originates in the left atrium of the heart and travels to the distal aorta. The term 'saddle' refers to the clot’s tendency to lodge at the trifurcation of the aorta, where it splits to supply the hind limbs. This obstruction creates a complete cessation of blood flow to the rear extremities, resulting in a medical crisis.
The Relationship Between Cardiac Function and Clot Formation
In most feline cases, this condition is secondary to chronic heart muscle changes, particularly Restrictive Cardiomyopathy. When the heart structure is compromised, the turbulence of blood flow causes the blood to clot within the heart chambers. These clots are unstable and can be released into systemic circulation at any time. If the cardiac output is poor, the likelihood of such events increases significantly.
Clinical Signs and Acute Symptom Recognition
Recognizing a saddle thrombus is usually straightforward due to its sudden and painful nature. Owners often notice the cat suddenly dragging its hind legs, crying out in vocal distress, or attempting to hide. The primary indicators include:
- Acute, non-weight-bearing paralysis of one or both hind limbs.
- Extreme vocalization due to pain.
- Cold, pale, or cyanotic paw pads.
- Absent or severely diminished femoral pulses.
- Rapid, labored breathing (often signaling congestive heart failure).
Why This Is a Critical Emergency
Because of the massive ischemia (lack of blood flow) to the legs, muscle damage occurs within hours. This leads to the release of toxins and potassium into the bloodstream, which can cause cardiac arrest or severe systemic acidosis. If you suspect your cat is suffering from FATE, immediate transport to an emergency clinic is vital.
Diagnostic Protocols in Emergency Medicine
Veterinarians perform several diagnostic steps to confirm the blockage and evaluate the heart's function. This typically involves an echocardiogram to check for Unclassified Cardiomyopathy or other structural heart diseases. Blood pressure monitoring is crucial, especially in patients with Systemic Hypertension, to ensure perfusion is optimized. Biochemical panels are used to monitor electrolyte levels, particularly potassium, which spikes when muscles are damaged from the clot.

Recovery from a saddle thrombus is challenging. Many cats have residual motor deficits, and the risk of a second clot remains a constant threat. Long-term care involves daily administration of antiplatelet or anticoagulant medications and regular monitoring of the heart using echocardiography to track the progression of the underlying Unclassified Cardiomyopathy.
