The primary issue is diastolic dysfunction; while the heart may contract normally, the ventricular walls are too stiff to accommodate adequate blood volume during the filling phase.
Endocardial Fibrosis and Ventricular Stiffening
In many cases, RCM is associated with endomyocardial fibrosis, where the inner lining of the heart becomes thickened or scarred. This fibrotic change restricts the ventricle's ability to stretch, leading to increased filling pressures. These pressures eventually back up into the pulmonary veins, causing fluid to accumulate in the lungs (pulmonary edema) or the thoracic cavity (pleural effusion).
