Feline Hepatic Lipidosis, often referred to as fatty liver syndrome, is a severe metabolic derangement where hepatocytes (liver cells) become overloaded with triglycerides. Unlike some conditions like Feline Plasma Cell Pododermatitis, which are localized, FHL is a systemic emergency that impacts the cat's entire metabolic pathway. When a cat stops consuming enough calories, the body shifts to burning stored fat; the liver is tasked with converting this fat into energy, but the feline liver is uniquely ill-equipped for high-volume lipid metabolism, leading to a bottleneck of fats within the liver tissue.

The Role of Secondary Triggers

In many cases, FHL is secondary to an undiagnosed condition. A cat may stop eating due to dental pain from a Feline Odontoclastic Resorptive Lesion, chronic digestive distress, or systemic stress. While a cat might exhibit skin irritations like those seen in Feline Eosinophilic Granuloma, these superficial issues rarely cause the degree of anorexia necessary to trigger FHL, but stress is a known catalyst.

Clinical Signs of Hepatic Dysfunction

Recognizing the warning signs early is the difference between life and death. The condition often begins with a 'picky eater' phase that progresses to total food refusal.

Physical Manifestations

  • Icterus (Jaundice): The eyes, skin, and gums take on a distinct yellow tint due to liver bile obstruction.

  • Ptyalism (Drooling): Often associated with severe nausea.

  • Muscle Wasting: Rapid loss of weight in the back and limbs despite a rounded abdomen.

  • Lethargy: The cat becomes remarkably quiet and unresponsive.

Emergency Indicators

If your cat begins to vomit repeatedly, exhibits abnormal neurological behavior (head pressing or circling), or refuses water, seek immediate emergency care. These signs indicate that Feline Hypertensive Retinopathy or hepatic encephalopathy may be developing as complications.

Diagnostic Pathways in Veterinary Medicine

Diagnosis involves a multi-pronged approach. Veterinarians often run a biochemistry profile to look for elevated ALT, ALP, and bilirubin. While a Feline Mast Cell Tumor might cause specific blood profile changes, FHL specifically shows an isolated elevation of ALP relative to GGT. Ultrasound is frequently used to visualize a hyperechoic liver, which suggests significant fat accumulation.

Aggressive Nutritional Support

The cornerstone of treatment is caloric intake. An esophageal feeding tube is frequently placed because it allows for the precise, frequent delivery of high-protein, high-calorie liquid diets without force-feeding, which causes extreme stress. Treating the liver failure is impossible without feeding the cat.

Managing Chronic Complications

Long-term, owners must be vigilant about recurrence. If a cat previously suffered from Feline Primary Hyperaldosteronism or other endocrine disorders, these must remain strictly managed to prevent the stress that leads to recurring bouts of anorexia. Unlike Feline Intestinal Adenocarcinoma, which requires surgical intervention or oncology protocols, FHL management is centered on nutrition and metabolic stabilization.
