Understanding Feline Odontoclastic Resorptive Lesions (FORL)
Feline Odontoclastic Resorptive Lesions are progressive conditions where a cat's immune system mistakenly targets the tooth structure for destruction. Unlike human cavities, which are caused by bacteria and plaque, FORLs involve the body literally dissolving its own teeth. The condition is categorized into Type 1, involving localized inflammation and periodontal disease, and Type 2, which involves the replacement of the tooth root with bone-like material (ankylosis).
The Progression of Dental Resorption
Initially, the lesion appears as a small defect at or below the gumline. As the odontoclasts continue their activity, the tooth loses structural integrity. Over time, the pulp chamber becomes exposed to the oral cavity, which is intensely painful. In some cases, the crown of the tooth may simply fall off, leaving roots embedded deep within the jawbone, which must be addressed via Urolithiasis protocols or standard surgical extraction to prevent secondary infection.
Identifying Clinical Signs and Warning Signs
Recognizing the subtle behavioral changes in a cat experiencing dental pain is critical. While some cats show obvious signs like dropping food or facial pawing, many remain quiet, suffering in silence.

When to Seek Emergency Care
If your cat stops eating entirely or displays extreme facial swelling, seek immediate veterinary intervention. While Feline Megacolon presents with different symptoms, the acute distress of a cat in severe oral pain mimics other systemic emergencies. If you notice a red, inflamed bump near the gums or if your cat is pawing at their mouth aggressively, this is a sign of an exposed nerve that requires urgent attention.
Causes and Risk Factors
The exact etiology of tooth resorption remains unknown, though several theories exist. Some researchers point to dietary changes, specifically high Vitamin D intake, while others highlight the role of chronic oral inflammation. Genetics play a significant role, as certain breeds show higher susceptibility. It is important to distinguish this from other systemic issues like Feline Primary Hyperaldosteronism, which, while unrelated, often presents in older cats, the same demographic most affected by FORLs.
Diagnostic Protocols
Veterinarians utilize a multi-modal approach for diagnosis. A visual examination under anesthesia is the gold standard because the lesions are often hidden. Full-mouth dental radiographs are mandatory to evaluate the status of the roots. During this exam, the vet will differentiate FORLs from Feline Mast Cell Tumor of the oral cavity or Feline Intestinal Adenocarcinoma metastases, which may appear as gingival masses.
Treatment and Surgical Intervention
The treatment for a tooth with advanced resorption is extraction. There is no medication that can reverse the process. If the tooth has already undergone significant replacement resorption (Type 2), a crown amputation may be performed where the root is left behind if it has fully integrated with the bone. Pain management, including non-steroidal anti-inflammatory drugs and local nerve blocks, is essential during and after surgery.
