Understanding the Feline Eosinophilic Granuloma Complex (FEGC)
The Eosinophilic Granuloma Complex is a heterogeneous group of cutaneous inflammatory conditions. It represents an abnormal reactive process involving the eosinophil, a type of white blood cell involved in allergic responses. The condition is broadly classified into three specific clinical types: the Eosinophilic Plaque, the Indolent Ulcer, and the Linear Granuloma. While similar to other inflammatory dermatological issues like Feline Plasma Cell Pododermatitis, FEGC is uniquely driven by allergic sensitization.
The Three Clinical Faces of FEGC
The Eosinophilic Plaque
These lesions appear as well-circumscribed, raised, moist, and intensely pruritic patches. They are commonly found on the abdomen or medial thighs. Because they are intensely itchy, cats often traumatize these areas, leading to secondary infections. These must be differentiated from neoplasms such as Feline Mast Cell Tumor through diagnostic testing.
The Indolent Ulcer (Rodent Ulcer)
Typically located on the upper lip, these ulcers are often non-painful but can grow to cause significant tissue destruction. They start as a small, firm area that eventually erodes into a characteristic yellow-pink, ulcerated plaque. Unlike Urolithiasis, which causes internal distress, the indolent ulcer is primarily a visible, external concern.
The Linear Granuloma
These are firm, yellowish-pink nodules that typically appear as a long, thin band along the back of the hind legs. They may also appear in the mouth or on the chin. While sometimes painless, their physical presence can restrict movement or cause irritation.
Diagnostic Pathways and Differential Considerations
Veterinarians must approach these lesions with a structured diagnostic plan. Because symptoms can mimic other systemic diseases like Feline Primary Hyperaldosteronism (through systemic inflammation) or localized infections, an accurate diagnosis is paramount. Diagnostic steps include:

Cytology: Sampling cells via impression smear or needle aspirate to look for high counts of eosinophils.

Biopsy: Histopathology is the gold standard to confirm the granulomatous nature of the lesion.

Elimination Diets: To rule out food allergies.

Parasite Control: Rigorous flea control, as flea saliva is a common trigger.
Management and Treatment Modalities
Treatment focuses on two fronts: controlling the immune response and removing the antigen. Corticosteroids remain the first-line treatment to provide rapid relief from inflammation and pruritus. In cases resistant to steroids, cyclosporine or other immunomodulatory agents may be used. Surgical excision is rarely recommended unless the lesion is extremely localized and causing significant mechanical dysfunction, as the immune response often triggers new lesions post-surgery. Environmental modifications, such as introducing hypoallergenic diets or enhancing flea prevention, are the pillars of long-term care.
