Equine laminitis is a complex, multi-factorial condition affecting the dermal-epidermal junction of the hoof. At the core of the disease is the failure of the laminae, which serve as the primary attachment point between the hoof wall and the third phalanx (pedal bone). When this interface is damaged by metabolic shifts or inflammatory mediators, the horse experiences excruciating pain. The disease is generally categorized into three phases: developmental, acute, and chronic. Unlike Colitis, which is localized to the gastrointestinal tract, laminitis is often the systemic manifestation of a broader metabolic crisis.
Endocrine-Driven Laminitis vs. Sepsis-Related Laminitis
The triggers for laminitis are diverse. Endocrinopathic laminitis is frequently associated with Pituitary Pars Intermedia Dysfunction (PPID) or Equine Metabolic Syndrome (EMS). In these cases, hyperinsulinemia is thought to be a primary driver of laminar cell dysfunction. Conversely, sepsis-related laminitis occurs due to systemic inflammatory responses, often following incidents such as grain overload, retained placenta, or severe cases of Hemorrhagic Gastroenteritis, where endotoxins circulate in the bloodstream and trigger peripheral vasoconstriction in the feet.
Clinical Indicators and Diagnostic Assessment
Recognizing the early symptoms of laminitis is vital. Horses often exhibit a 'sawhorse' stance, shifting weight to their heels to relieve pressure on the inflamed toe.
Physical and Imaging Diagnostics
Veterinarians diagnose laminitis through a combination of clinical signs and diagnostic imaging. Radiographs are essential to visualize the position of the pedal bone relative to the hoof wall. Unlike the diagnosis of a Foreign Body Ingestion which relies on radiographic density of an object, laminitis imaging focuses on measuring the 'founder distance' and the angle of rotation of the third phalanx.
Therapeutic Interventions and Management Protocols
Treatment must be aggressive and immediate. Cryotherapy (icing the feet) is the gold standard during the acute inflammatory phase to limit metabolic demand and enzyme activity within the foot. Analgesia, including non-steroidal anti-inflammatory drugs (NSAIDs), is essential to provide comfort. In cases where the condition is linked to a systemic metabolic imbalance, owners must manage the underlying condition, much like a patient with Protein-Losing Enteropathy requires rigorous dietary and metabolic management. Mechanical support, such as frog support pads or specialized farriery, is often required to redistribute weight off the hoof wall and onto the sole or frog.
Long-Term Management and Structural Salvage

The goal is to stabilize the pedal bone and promote the growth of a healthy, properly connected hoof capsule. This is a slow, tedious process that demands strict confinement, therapeutic trimming, and consistent monitoring of metabolic health to prevent future episodes.
